n n n Care Partner
– 2nd & 3rd Shift
– HaborChase n nn Part Time positions available on 2nd and 3rd shifts.

nn nn Position Summary: nn nn Provides residents assistance with Activities of Daily Living (ADL) while recognizing the importance of their dignity and respect at all times.

nn nn n Essential Functions: nn nn ▪ Provides quality care and assistance to residents in accordance with the resident philosophy and policies and resident rights n ▪ Assists residents in meeting their physical, emotional and spiritual needs n ▪ Adheres to each resident’s Level of Care (LOC), notifies supervisor of any changes in resident’s condition n ▪ Answers and responds to resident calls in a timely manner n ▪ Assists residents with Activities of Daily Living (ADL) n ▪ Checks residents every two (2) hours and reports findings n ▪ Completes assignment sheets daily.

n ▪ Encourages residents to attend meals in the dining room by escorting to and from Dining room n ▪ Assists residents with meals in ways that meet their individual needs and desires, while being cognizant of changes in appetites and physical abilities to eat and drink n ▪ Assists with cleaning and setting the dining room as directed by supervisor n ▪ Assists with cleaning and tidying resident’s room as directed by supervisor n ▪ Assists with resident’s personal laundry as directed by supervisor n ▪ Establishes and maintains good relationships with residents and their families n ▪ Disposes of all incontinent products according to OSHA regulations and Company policy n ▪ Practices proper body mechanics when lifting following safety guidelines n ▪ Responds to resident emergencies n ▪ Works assigned schedule n ▪ Follows proper procedure for clocking in and out for shifts n ▪ Follows guidelines for uniforms and wears name badge daily n ▪ Follows policy on signing equipment in and out, and uses Company assigned equipment properly as intended n nn nn Non-Essential Functions: n ▪ Leads by example exhibiting the CORE Values through servant leadership n ▪ Encourages teamwork and promotes company philosophy n ▪ Attends required community meetings and participates Life Enrichment activities when able n ▪ Completes all required courses in adherence with HRA University n ▪ Is prompt and able to perform the required duties of the position on a regular, predictable basis n ▪ Becomes familiar and understands how to report a missing person using the Safe Return program or equivalent n ▪ Becomes familiar and understands the steps for fire evacuation n ▪ All associates are responsible for maintaining a safe and secure environment for all community residents nn nn n Qualifications/Skills/Educational Requirements: n ▪ Must have a high school education n ▪ Non-Certification with proper training, or possession of valid state certification as C.N.A.

n ▪ 1 – 3 years experience in a senior care community or nursing home environment preferred n ▪ Has a good understanding of how Dementia and Alzheimer’s Disease can affect behavior n ▪ Able to communicate effectively with all levels of management, associates, residents, family members, and outside contacts n ▪ Must possess a passion to work with and around senior citizens nn n n
