Resident Wellness AssistantJob DescriptionJob SummaryThe Resident Wellness Assistant works with the Independent Living residents at Linus Oakes to assist with obtaining answers to health or insurance questions, perform minor wound treatment, check blood pressure, keep accurate records, ensure resident and work environment safety, and keep Health Office equipment stored and maintained in working condition.Department: Independent LivingReports to: Executive DirectorEssential Job Specific Duties/Responsibilities1.

Assists residents in finding a primary care provider, dentist, or other health care providers.2.

Assists residents with arranging a private caregiver or Home Health/Hospice if resident needs this level of care.3.

Follows up with resident after hospital stay, fall, or illness.4.

Performs treatment of minor wounds.5.

Assists residents in obtaining answers to health or insurance-related questions.6.

Checks blood pressures for residents as requested.7.

Arranges for flu shots or shingles vaccines for residents.8.

Assists in administering medications such as ear and eye drops according to hospital policies/procedures to ensure resident safety.

(Does not routinely administer medications to residents.)9.

Keeps accurate and detailed records of resident contacts made.10.

Provides health related educational opportunities for groups of residents.11.

Provides for and follows precautions to encourage resident safety related to fall prevention.12.

Actively participates in the maintenance of a safe work environment.

Must be able to respond to resident emergency situations.13.

Ensures that Linus Oakes Health Office equipment is available, in proper place, and maintained in working condition.14.

Utilizes safety equipment in accordance with policies to ensure own and coworkers’ safety needs.15.

Assists in providing transportation for residents to various appointments and activities as requested.16.

Demonstrates ability in the care and handling of geriatric patients, including special consideration of their specific needs.17.

Satisfies all educational in-service requirements mandated by CHI Living Communities, the department, external accrediting, and regulatory agencies.a.

Attends all mandatory in-service programs.b.

Adheres to facility policies and procedures.c.

Completes yearly online education program by deadline.d.

Attends employee meetings on a regular basis.In addition to the essential job specific duties listed above, the Resident Wellness Assistant shall be required to perform all duties (essential and non-essential) in a manner consistent with the mission and values (compassion, excellence, inclusion, integrity, collaboration) of CHI Living Communities and will be evaluated on such basis.

Furthermore, every employee must abide by all campus, departmental, and safety policies, rules, and regulations.CHI Living Communities has the right to change the job specific duties and specifications required for the position of Resident Wellness Assistant from time to time without prior notice.Job RequirementsEducation: High School Diploma or GED.

Current RN or LPN in the State of Oregon.Experience: Two years clinical experience required.

Experience working with senior population preferred.
